Seeking a Full-Time LPN to provide end-of-life care to hospice patients in their homes or our contracted facilities. The LPN will also be asked to perform the duties of an On-Call Runner providing wound care.
Responsibilities: Under the supervision of the team director will provide continuous care services, routine patient visits, in-home respite care and personal care for patients.
As a Runner the LPN will perform emergency assessment visits as directed by the team director. Coordinates the patient plan of care with the interdisciplinary team.
Work schedule will be Monday through Friday 8:30am to 5pm.
LPN licensed in NY state
2+ years of LPN experience
Reliable motor vehicle.
Ability to work with limited supervision.
Strong interpersonal skills.
Collaborative and team-oriented personality.
Reliable motor vehicle required in order to reach patients on our program.
