Job Summary
We are seeking a compassionate, dependable, and patient-focused Licensed Practical Nurse to join our primary care team. The LPN will work closely with physicians, advanced practice providers, medical assistants, and care-coordination staff to support the delivery of safe, efficient, and high-quality care.
This position is responsible for providing direct nursing care, assisting with patient assessment and treatment, supporting chronic-disease management, responding to patient concerns, and helping ensure that patients receive appropriate follow-up care. The ideal candidate will be organized, clinically competent, comfortable working independently within established protocols, and committed to serving patients who may experience financial, transportation, social, or other barriers to care.
This position is scheduled for no more than 32 hours per week, based on organizational and patient-care needs.
Duties
- Assist providers with examinations, treatments, minor procedures, wound care, and other clinical services.
- Administer medications, injections, immunizations, and treatments as ordered and within the LPN’s authorized scope of practice.
- Respond to patient telephone calls and electronic messages using approved clinical protocols, escalating concerns to a provider when appropriate.
- Reinforce provider instructions and educate patients regarding medications, chronic conditions, preventive care, testing, and follow-up needs.
- Support patients with diabetes, hypertension, asthma, cardiovascular conditions, and other acute or chronic healthcare needs.
- Review laboratory and diagnostic-testing workflows to help ensure results are received, documented, routed to the provider, and communicated as directed.
- Assist with prescription-refill requests in accordance with provider instructions and organizational refill protocols.
- Conduct outreach to patients who require follow-up after hospitalization, emergency department treatment, missed appointments, abnormal testing, or changes in their care plan.
- Work with the Care Coordinator and Referral Specialist to address outstanding referrals, preventive screenings, care gaps, transportation barriers, and other patient needs.
- Document all nursing services, patient contacts, treatments, and follow-up activities accurately and promptly in the electronic health record.
- Maintain medication, vaccine, clinical-supply, and emergency-equipment logs as assigned.
- Follow infection-prevention, medication-safety, patient-identification, privacy, and quality-assurance procedures.
- Assist with in-office medication-dispensing processes in accordance with organizational policy and provider oversight.
- Participate in team meetings, clinical training, quality-improvement activities, and emergency-preparedness exercises.
- Perform other nursing and clinical-support duties as assigned.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS (Basic Life Support)
- Proven experience working with diverse populations
- Demonstrated proficiency in medical documentation, ICD coding (ICD-9/ICD-10), and EMR/EHR systems such as Athena or EClinicalWorks
- Skilled in medication administration, venipuncture, phlebotomy, and aseptic techniques
- Strong communication skills for caregiver interaction and patient education; ability to work effectively within multidisciplinary teams

Pay: $23.00 - $25.00 per hour
