This role is built on assertive engagement, comprehensive care coordination, and real-world support for individuals navigating substance use disorders and co-occurring mental health issues. What You Will Do
- Direct Client Care: Provide assessments, service planning, and resource linkage. Support clients in achieving independence in daily living, employment, and social functioning.
- Field Engagement: Conduct frequent community-based visits (homes, workplaces, etc.) and maintain daily touchpoints with assigned clients.
- Care Coordination: Act as the central hub for medical providers, mental health professionals, and family systems.
- Clinical Administration: Maintain timely documentation and participate in weekly clinical team meetings.
What We Are Looking For
- Education: Bachelor’s degree in Social Work, Psychology, or related field (Master’s preferred).
- Experience: Minimum of 2 years working with substance use or mental health disorders; community-based experience preferred.
- Licensure: Preferred certification (e.g., CADC, LCAS, LCSW, LMHC, CPRS).
- Logistics: Reliable personal transportation is strictly required for daily outreach.
Pay: $40,000.00 - $45,000.00 per year
