Location: Boardman, OH (Main Campus)
Schedule: Full-time. Monday – Friday. Day shift.

The Role
You will assist patients recently discharged from acute care. You will gather clinical data and help prevent hospital readmissions.
Key Responsibilities:
- Patient Assessments: Conduct post-acute phone assessments. Identify barriers to patient recovery.
- Care Coordination: Review discharge instructions, medications, and referrals.
- Scheduling: Facilitate and schedule face-to-face follow-up visits.
- Documentation: Log all patient calls accurately in the EMR system.
- Compliance: Maintain patient privacy and strictly follow regulatory standards (OSHA, ODH).
Qualifications
- License: Active Ohio RN license.
- Education: BSN degree is preferred.
- Skills: Strong critical thinking and time management.
- Communication: Clear, professional, and empathetic phone skills.
