ACM Home Health is seeking to hire a PRN LPN /RN Nurse.

Position responsibilities may include, but are not limited to:
- Patient education of medications, disease processes, safety and more.
- Provide wound care, medication management, medication administration and other treatments as per physician’s orders.
- Communication and coordinate with case manager on patient status and care needs.
Local travel is required so you must have reliable transportation and be comfortable driving in all weather conditions. Mileage is reimbursed.
Ideal Candidate Qualities:
- Excellent personal skills required to communicate and coordinate care with patients, families & other health care professionals
- Great Communication & time management skills
- Conscientious, thorough, & able to work independently
- Home Care experience preferred. Will train the right candidate.
