Position Description:
At Saint Michaels Hospice, we are looking for compassionate and experienced Registered Nurses to join our team and provide exceptional care to our patients at our Houston/ Kingwood location. In this role, you will be responsible for delivering high-quality hospice care, coordinating with interdisciplinary teams, case management end to end for all patients, and ensuring compliance with all regulatory requirements.
Responsibilities:
- Provide direct patient care in accordance with hospice care plans.
- Collaborate with physicians, social workers, and other healthcare professionals.
- Conduct patient assessments and create personalized care plans.
- Administer medications and treatments as prescribed.
- Educate patients and families about care plans and symptom management.
- Document patient care activities accurately and timely.
Qualifications:
- Valid RN license in the state of Texas.
- Minimum of 1-2 years of nursing experience (hospice experience preferred).
- Strong clinical and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic nature.
