What You'll Do
As a Case Manager, you will assess participant needs, coordinate services, and support individuals in achieving their recovery and life goals. The Case Manager plays a key role in implementing treatment plans, advocating for participants, and connecting them with community resources. You will work collaboratively with a multidisciplinary team to ensure person-centered, high-quality care that promotes independence and overall well-being.
Your responsibilities may include:

Service Coordination & Client Support

Assess participant needs and identify appropriate services and supports

Develop, implement, and monitor individualized recovery and treatment plans
-
Connect participants to community resources including healthcare, housing, and social services
-
Support participants in achieving goals related to independence and community integration
-
Transport participants to appointments as needed

Collaboration & Teamwork
-
Participate in multidisciplinary team planning and service coordination
-
Collaborate with clinical, medical, and external providers
-
Build and maintain relationships with community agencies
-
Advocacy & Program Support
-
Advocate for participants to ensure access to needed services
-
Identify service gaps and recommend improvements
-
Documentation & Compliance
-
Complete assessments, service documentation, and reports in a timely manner
-
Maintain compliance with organizational policies, HIPAA, and regulatory standards

Attend required trainings, meetings, and supervision

Qualifications
-
Minimum 2 years of experience supporting individuals in behavioral health, IDD, addiction recovery, or similar populations
-
Basic to intermediate computer skills, including documentation and data entry
-
Ability to work in the community and transport participants to appointments
-
Strong communication and interpersonal skills
-
Ability to maintain confidentiality and adhere to ethical standards
Additional Qualifications
-
Bachelor’s degree in Human Services or a related field (or equivalent experience)
-
Previous case management experience
-
Knowledge of community resources and service systems
-
Strong organizational and problem-solving skills
-
Crisis intervention and conflict resolution experience
-
Ability to work collaboratively in a team environment
Schedule

Flexibility to work evenings, weekends, or adjusted schedules as needed
