Description:
JOB SUMMARY
Assesses the psycho-social needs of adult and adolescent patients. Provides on-going follow-up and support; serving as an advocate with identification and referral to community services for patients and their families. Maintains knowledge of available community resources and services. Works collaboratively with other team members in clinical settings, patient conferences and meetings. Provides counseling sessions with adult and adolescent patients.
This position is 48 hours per pay period
ESSENTIAL FUNCTIONS

Develops and maintains social service program, drafting supporting policies, procedures, and guidelines for the program.

Works closely with all members of the health care team in development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.

Provides individual and group therapy to patients and families following treatment plan goals.

Accurately assesses and documents clients presenting problems, history, symptomology, mental status, diagnosis and prescribes initial interventions in a timely manner.

Meets with client/patients to assist in developing specific, realistic, and measurable treatment goals and objectives.

Complete psychosocial evaluations and validate family and support systems for patients.

Maintain knowledge and understanding for psychosocial evaluation regulatory requirements patients.

Provide counseling, support and follow-up to recommendations with identification and resolution to barriers contributing to access, adherence and financial issues.

Contribute to on-going treatment plans, compliance with medications, treatment regimens; offering solutions with facilitation, follow-up and monitoring for psychosocial plans.

Maintains accurate and timely records, including clinical and progress notes, according to department policy.

Participates in planning and implementing quality improvement activities.

Maintains up-to-date knowledge of field by participating in continuing education activities.

Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies, and others.

Adheres to all department/hospital policies.

Supervises social work students/ as approved by the director and administrator.

Social service intervention planning for patient's needs for post-hospital care.

Provides coverage for Social Service Director's absence.

Other duties as assigned.
Requirements:
JOB REQUIREMENTS

Must have a current/active and in good standing license in Ohio for LISW.

Master's Degree in Social Work.

At least two years of social work experience in a health care setting and one to three years related experience with mental health or counseling preferred.

Experience providing services to adults and adolescents preferred

Physical, sensory, and cognitive abilities sufficient to perform essential functions. Good interpersonal and written and verbal communication skills.

Must be caring, compassionate and have a strong desire to provide aid and make an impact.
