WHAT WE BRING TO THE TABLE
As we bring our proven care model into this new market, we're offering an opportunity that combines strong earning potential with long-term growth. This contract role provides an annual range of $150,000 to $200,000, giving you the chance to step into a high-impact position right from the start.

WHAT WE NEED FROM A POST-ACUTE AND LONG-TERM CARE APP

1+ years of experience in post-acute/SNF/LTC
-
Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)

Post-acute, long-term, and/or skilled nursing facility experience
WHAT YOUR DAY ENTAILS
No two days are the same, especially as we establish a new footprint in the area, but here's what you can expect:

Providing high-quality care for patients managing chronic and acute conditions

Working alongside an attending physician to guide treatment decisions

Partnering with the Directors of Nursing, administrators, and care teams to coordinate patient care

Helping establish strong clinical practices as MedCap Health enters the community
