Progressive and well established Adult Day Health facility is looking for a qualified Program Nurse to service a culturally rich and diverse population in Hyde Park, MA. Bi-lingual Spanish candidates strongly encouraged to apply. This position requires an individual who is proactive, positive, empathetic and active. Computer skills are required. The Program Nurse is responsible for coordinating and delivering nursing services within the Adult Day Health setting. The role includes assessing participant needs, developing and implementing care plans, providing health education, monitoring patient progress, maintaining accurate medical records, and collaborating with healthcare providers and multidisciplinary teams. The Program Nurse also will ensure compliance with clinical standards, support program goals, and promote quality patient care while serving as a resource for clients, families, and staff.
Required license or certification:
Must have current Massachusetts Registered Nurse License
