Licensed Practical Nurse
Field LPN Part-Time: Monday through Friday, 5:00 PM – 9:00 PM
East End Hospice is seeking a Part-Time LPN to join its Interdisciplinary care team serving patients at home and in other care settings in the community. East End Hospice serves the eastern region of Long Island from Patchogue to Montauk.
Responsibilities of an LPN include:
- Under the direction of the RN, assist in the implementation of the plan of care.
- Assist in identifying patient problems and implementing necessary interventions, including pain and symptom management.
- Respond to the physical, psychosocial, emotional, cultural, and other special needs of terminally ill patients and their families.
- Provide anticipatory guidance to patients/families to assist them in preparing for and coping with the events as they occur throughout the disease process.
- Assist patients with care needs as necessary.
Qualifications:
- Ability to relate well to terminally ill patients and their families.
- Ability to assess patient/family needs and to be sensitive to those needs.
- Willingness to function as a member of the interdisciplinary team.
- Be familiar with the hospice concept and related nursing issues.
Requirements:
- Valid license and current registration to practice as a Licensed Practical Nurse in New York State.
- 2+ recent Medical-Surgical or Hospice & Palliative experience preferred
- Experience with Electronic Medical Records a plus.
A valid New York State driver’s license and proof of current automobile insurance are required for all positions. East End Hospice offers competitive salaries and a supportive working environment.
Pay: $34.00 - $39.00 per hour
