We are seeking a compassionate and organized Social Services Coordinator to support residents in a long-term care setting. This role focuses on resident advocacy, discharge planning, admissions coordination, and ensuring compliance with state and federal regulations.
Key Responsibilities:
- Complete documentation in PointClickCare (PCC), including admission, quarterly, interim, and discharge notes
- Complete MDS Sections A1500, C, D, E, and Q per schedule
- Conduct social service assessments, including intake and PHQ forms
- Provide ongoing emotional support and assist residents with adjustment and psychosocial needs
- Participate in interdisciplinary care plan meetings with residents and families
Discharge Planning:
- Coordinate safe discharges, including home care referrals, community programs, and durable medical equipment
- Schedule follow-up appointments with primary care providers
- Complete discharge documentation and submit required forms (ADS Portal, W10, etc.)
- Maintain hospital transfer logs and required reports
Admissions:
- Review and process referrals from hospitals and electronic systems (CarePort, Epic)
- Coordinate admissions, including paperwork, insurance verification, and legal documentation
- Complete admission assessments and documentation in PCC
- Prepare charts and communicate admission details to all departments
Compliance & Reporting:
- Ensure PASRR compliance and maintain required records
- Manage grievance documentation and follow-up
- Attend QAPI and medical staff meetings and submit reports
Qualifications:
- Bachelor’s degree in Social Work or related field preferred
- Experience in long-term care, skilled nursing, or healthcare setting preferred
- Knowledge of PointClickCare (PCC) and MDS process preferred
- Strong communication, organizational, and interpersonal skills
- Ability to multitask and work collaboratively with an interdisciplinary team
