Overview
We are seeking a dedicated and experienced Home Care Aide Supervisor to lead and coordinate in-home care services for our clients. The ideal candidate will possess strong leadership skills, a compassionate approach to patient care, and extensive experience in senior and disability care settings. This role involves overseeing caregiving staff, ensuring quality care delivery, and maintaining compliance with healthcare standards such as HIPAA. The Home Care Aide Supervisor plays a vital role in supporting clients' independence and well-being within their homes and community environments.
Duties
- Supervise and coordinate home care aides to ensure delivery of personalized care plans tailored to each resident’s needs.
- Assist with and oversee activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance.
- Conduct patient observation to identify changes in condition or behavior, especially in individuals with dementia, Alzheimer's, or developmental disabilities.
- Collaborate with the Case Manager to develop, implement, and update individualized care plans.
- Provide training and ongoing support to caregiving staff on policies, care updates, and needed skills.
- Ensure proper documentation of patient care activities, incident reports, and compliance with healthcare standards.
- Maintain a clean and safe environment by overseeing cleaning routines and infection control practices.
- Coordinate with families, healthcare providers, social workers, and community resources to optimize patient outcomes.
- Follow up on care or services provided by checking the caregiver and client routinely.
- Responsible for making sure that care notes are completed based on standard documentation and gathered on a timely manner.
- Do a routine and random visits or phone calls to clients and caregivers.
Experience
- Prior experience working in assisted living facilities, nursing homes, hospice care, or home health environments is preferred.
- Demonstrated expertise in dementia care, Alzheimer's care, memory care, or working with individuals with autism or developmental disabilities.
- Proven ability to supervise caregiving staff effectively while maintaining high standards of patient safety and satisfaction.
- Knowledge of medication administration protocols and vital signs monitoring.
- Familiarity with HIPAA regulations and confidentiality standards in healthcare settings.
- Experience with developing care plans and providing direct support for residents’ physical and emotional needs.
- Strong background in social work or long-term care is advantageous.
- Certifications such as CPR, first aid, or nursing assistant credentials are highly desirable. Pay: $28.00 - $33.00 per hour
