We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team for the 7:00 AM – 3:00 PM shift. This role is essential in providing high-quality care to our residents in a personal care and memory care setting, supporting both their physical health and emotional well-being.
Key Responsibilities
- Administer medications and treatments as prescribed by physicians
- Monitor residents’ health status and report changes to the care team
- Provide direct nursing care and assistance with activities of daily living (ADLs) as needed
- Support care staff to ensure quality resident care
- Maintain accurate and timely documentation in accordance with state regulations
- Communicate effectively with residents, families, and interdisciplinary team members
- Ensure a safe, clean, and supportive environment for residents
- Respond to emergencies and follow facility protocols
Qualifications
- Current and valid LPN license in the state of employment
- Experience in personal care, assisted living, or memory care preferred
- Knowledge of dementia care practices is a plus
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate, patient, and resident-centered approach to care
Schedule
- Full-Time
- Day Shift: 7:00 AM – 3:00 PM
- Includes rotating weekends and holidays as needed
