Job Overview
Senior Care Navigator Group is seeking compassionate, dependable, and experienced Licensed Practical Nurses (LPNs) to provide high-quality, personalized nursing care to clients in their private homes. The LPN will work under the direction of the Director of Nursing and follow each client’s individualized plan of care. Responsibilities include administering medications and treatments, monitoring changes in clients’ health conditions, maintaining accurate documentation, and communicating with families and the care team. The ideal candidate is professional, attentive, reliable, and committed to supporting each client’s safety, comfort, dignity, and independence at home.
Responsibilities
- Provide skilled nursing care to clients in their homes according to individualized plans of care.
- Administer medications and treatments as ordered by the physician or authorized provider.
- Monitor clients’ health status, vital signs, symptoms, and response to treatment.
- Perform wound care, catheter care, medication management, and other approved nursing procedures.
- Document all services accurately and promptly in the electronic health record.
- Report changes in a client’s condition to the supervising RN and appropriate healthcare providers.
- Communicate professionally with clients, families, caregivers, and the interdisciplinary care team.
- Educate clients and caregivers on medications, treatment plans, safety, and disease management.
- Follow infection-control practices, HIPAA requirements, and agency policies and procedures.
- Respond appropriately to emergencies and complete incident reports when required.
- Participate in care conferences, staff meetings, training, and quality-improvement activities.
- Maintain a compassionate, respectful, and client-centered approach to care.
- Proven experience working as an LPN in diverse healthcare settings such as private homes, nursing homes, or outpatient clinics.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and documentation standards within EMR/EHR systems like Epic or Athenahealth.
- Skilled in performing clinical procedures
- Experience with specialized care areas such as geriatrics, memory care (including Alzheimer’s/dementia), hospice services, pediatric or neonatal units.
- Ability to handle emergency situations efficiently using skills like CPR and basic life support (BLS), with experience in triage or urgent/emergency care environments preferred.
- Familiarity with long-term care regulations and working with individuals with disabilities or developmental challenges.
- Excellent caregiver communication skills to effectively coordinate with patients’ families and multidisciplinary teams while maintaining clinical confidentiality policies.
Job Qualifications and Requirements
- Current, unrestricted Florida Practical Nurse license.
- Graduate of an approved practical nursing program.
- Current CPR/Basic Life Support certification.
- Minimum of one year of nursing experience preferred; home health experience is a plus.
- Ability to pass a Level 2 background screening and drug screening.
- Current physical examination and negative tuberculosis screening, as required.
- Valid Florida driver’s license, reliable transportation, and proof of automobile insurance.
- Knowledge of medication administration, infection control, and clinical documentation.
- Ability to recognize and report changes in a client’s condition.
- Strong communication, organizational, and time-management skills.
- Ability to work independently while following the direction and supervision of a registered nurse.
- Comfortable using electronic health records and mobile documentation systems.
- Ability to safely perform the physical duties of the position, including lifting, repositioning, and assisting clients.
- Must be dependable, compassionate, professional, and committed to protecting client dignity and confidentiality.
