SUMMARY OF RESPONSIBILITIES:
CLS Worker will assist the individual to increase or maintain personal self-sufficiency, facilitating goals of community inclusion and participation. Will assist individuals served gain access to participation in recreational, volunteer, and other community based activities of their choosing. May participate in the interdisciplinary team meetings and person-centered planning process. Assist, prompt, and support individuals served in activities identified in the Individual Plan of Service (IPOS). Will assist with preserving the health and safety of individuals served to support the most integrated and independent setting. CLS activities will focus on improving independence and enhancing recovery.
CLS Worker assigned to BHH will coordinate and provide access to community social supports and natural supports. Will attend weekly BHH care team meetings to review and discuss active cases. BHH CLS will identify community resources for BHH enrollees to utilize and maximize wellness. Will assist with referral tracking, implement wellness and prevention initiatives, facilitate health education groups, and provide education on health conditions, including strategies to implement care plan goals. Additionally, BHH CLS will coordinate and provide access to chronic disease management, including self-management support.
Qualifications
Education:
High school diploma or GED preferred.
Experience:
Experience working with individuals with intellectual/developmental disabilities (I/DD) or mental health needs is preferred. Must be able to communicate effectively with individuals, families, natural supports, and providers. Lived experience with mental illness, developmental disabilities, and/or substance use disorders is valued.
Other Requirements:
- Must be at least 18 years old.
- Valid driver’s license with acceptable driving record as determined by the agency.
- Ability to work a flexible schedule, including evenings, weekends, holidays, and occasional overnight travel.
- Ability to maintain positive public relations.
Essential Job Functions
- Provide services and supports as outlined in the IPOS.
- Follow agency policies regarding medication assistance and/or delivery.
- Assist individuals in identifying and maintaining volunteer opportunities as desired.
- Collaborate effectively with providers, families, natural supports, and case managers on assessments, treatment planning, and service delivery.
- Support individuals’ health and safety to promote independence in the most integrated setting.
- Prepare accurate progress notes, reports, and documentation in accordance with agency standards.
- Attend weekly BHH care team meetings (if assigned to BHH).
- Coordinate access to community social supports and natural supports for BHH enrollees.
- Implement wellness and prevention activities, facilitate groups, provide health education, and support access to chronic disease management services.
- Demonstrate knowledge of universal precautions, professional standards, and emergency protocols.
Non‑Essential Job Functions
- Perform other duties as assigned.
- Participate in quality improvement initiatives as requested.
- Provide personal transportation for agency business when required.
Physical Requirements
