Contract Registered Nurse (RN) – Host Home & Group Home Settings
We are seeking a compassionate and experienced Registered Nurse (RN) to provide nursing oversight and healthcare support for individuals receiving services in Host Home and Group Home settings. This is a contract position, not a full-time position, and is ideal for an RN who is comfortable providing community-based nursing services, completing certified nursing assessments, supporting caregivers, and coordinating healthcare needs.
The RN will support individuals with intellectual and developmental disabilities by monitoring their health needs, completing nursing assessments, coordinating medical care, providing nursing guidance, and maintaining accurate health-related documentation.
Essential Responsibilities
- Conduct comprehensive nursing assessments and health screenings for individuals receiving services.
- Complete required certified nursing assessments to identify and document individuals’ medical, nursing, functional, and health-related needs.
- Develop, review, and update nursing service plans and health-related documentation based on assessment findings.
- Monitor individuals for changes in health status and determine when additional medical evaluation or intervention is needed.
- Monitor medication administration and provide medication training to Host Home providers, caregivers, and staff as required.
- Coordinate medical appointments and communicate with physicians, specialists, pharmacies, and other healthcare providers.
- Review physician orders, medical recommendations, hospital discharge information, and other healthcare documentation.
- Provide health education and nursing training to Host Home providers, caregivers, and direct support professionals.
- Participate in interdisciplinary team meetings and person-centered planning meetings as needed.
- Assess and respond appropriately to health concerns, changes in condition, and medical emergencies.
- Promote person-centered care, health, safety, dignity, and independence.
- Ensure appropriate follow-up on identified health concerns and nursing recommendations.
- Maintain accurate, timely, and confidential nursing records, assessments, and care documentation.
- Ensure nursing services and documentation comply with applicable HCS requirements, state regulations, and agency policies.
- Provide ongoing nursing consultation and support to Host Home providers and Group Home staff.
Qualifications
- Current, unrestricted Texas Registered Nurse (RN) license.
- Minimum of one (1) year of nursing experience.
- Experience working with individuals with intellectual and developmental disabilities preferred.
- Experience in Host Home, Group Home, residential care, home health, community health, or long-term care preferred.
- Experience completing certified nursing assessments preferred.
- Knowledge of Home and Community-Based Services (HCS) requirements preferred.
- Strong clinical assessment, communication, organization, and documentation skills.
- Ability to work independently and manage a flexible contract caseload.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Current CPR certification.

This is a contract nursing position and is not a full-time position. Nursing services will be provided based on the needs of individuals served in Host Home and Group Home settings. The RN will complete required certified nursing assessments, provide nursing oversight, coordinate healthcare services, and support individuals, caregivers, and staff as needed.
Pay: $30.00 - $34.00 per hour
