Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation in the field setting.
- Administer medications and injections accurately following established protocols
- Provide wound care, catheterization, and sterile dressing changes while maintaining aseptic technique.
- Educate patients and caregivers on disease management, medication use, nutrition, and health maintenance strategies tailored to individual needs.
- Conduct triage assessments to determine urgency levels and coordinate appropriate interventions or referrals.
- Document all patient interactions thoroughly using electronic health record (EHR)
Skills
- Valid LVN license with current state licensure and certification in Basic Life Support (BLS).
- Familiarity with home & community care practices such as hospice care, long-term care facilities (nursing homes), assisted living communities, and post-acute settings.
- Ability to work independently in varied environments while maintaining high standards of infection control and HIPAA compliance.
- Excellent communication skills for effective patient education and collaboration with multidisciplinary teams.
Pay: $40.00 - $45.00 per hour
