Legato Living is a locally owned residential memory care company dedicated to providing exceptional care for individuals living with Alzheimer's disease and other forms of dementia. We operate three small-home memory care residences serving 29 residents in a warm, family-centered environment where residents are known, valued, and cared for with dignity.
We are seeking a compassionate, experienced, and hands-on Health Services Director to oversee the clinical operations of our three homes.
Position Summary
The Health Services Director is responsible for ensuring the delivery of high-quality resident care, maintaining regulatory compliance, supporting and educating care staff, and serving as the primary clinical resource for residents, families, physicians, hospice agencies, and other healthcare providers.
This position oversees the clinical care of approximately 29 residents across three residential memory care homes and requires an individual who is highly organized, clinically skilled, and passionate about serving older adults living with dementia.
Essential Responsibilities
Clinical Oversight
- Monitor resident health status and identify changes in condition.
- Complete resident assessments and reassessments as required.
- Develop, review, and update resident care plans.
- Ensure medication administration and treatments are completed accurately.
- Review documentation and charting for accuracy and compliance.
- Conduct routine audits of medication records and clinical documentation.
- Coordinate resident care with physicians, pharmacies, hospice providers, home health agencies, therapists, and other healthcare professionals.
- Attend and participate in resident care conferences as needed.
Staff Leadership & Education
- Provide clinical leadership and support to caregivers and medication aides.
- Train, mentor, and coach staff on dementia care best practices.
- Conduct nursing-related orientation and ongoing education for new and existing employees.
- Assist with competency evaluations and performance improvement initiatives.
- Promote a positive, professional, team-oriented culture.
Regulatory Compliance
- Ensure compliance with Nebraska assisted living regulations and company policies.
- Assist with surveys, inspections, and corrective action plans.
- Maintain clinical records in accordance with state and company requirements.
Family & Resident Support
- Serve as a primary clinical contact for families.
- Communicate changes in condition and care recommendations.
- Support residents and families through transitions, hospice involvement, and end-of-life care.
On-Call Responsibilities
- Participate in and manage clinical on-call responsibilities.
- Provide guidance to staff regarding resident concerns, medication questions, and changes in condition.
- Respond appropriately to urgent resident care situations.
Additional Duties
- Assist with admissions assessments and resident transitions.
- Support marketing and community outreach efforts as a clinical representative when appropriate.
- Provide hands-on resident care when needed.
- Collaborate with operational leadership to ensure excellent resident outcomes and family satisfaction.
Qualifications
- Active Nebraska RN or LPN license in good standing.
- Minimum of 3 years of experience working with geriatric populations.
- Minimum of 3 years of dementia or memory care experience required.
- Hospice or end-of-life care experience strongly preferred.
- Strong assessment and clinical decision-making skills.
- Excellent communication and leadership abilities.
- Ability to work independently and manage multiple priorities.
- Valid driver's license and reliable transportation.
- Professional references required.
