Position Summary
We are seeking a motivated and organized Home Health Office Coordinator / Intake Coordinator to join our Glendale office. This role is ideal for someone who thrives in a fast-paced healthcare environment and enjoys coordinating patient care while working closely with physicians, clinicians, referral sources, patients, and their families.
Home health experience is preferred, but we are happy to train the right candidate who is eager to learn and grow with our team.
Responsibilities
- Coordinate new patient referrals and intake
- Answer incoming phone calls and assist patients, families, referral sources, and physicians
- Schedule skilled nurses, therapists, and other clinicians
- Verify insurance eligibility and benefits
- Obtain insurance authorizations
- Coordinate with physicians' offices regarding orders and patient care
- Maintain accurate patient records and charts
- Perform data entry and ensure documentation is complete
- Order office and medical supplies
- Assist with payroll and timesheet tracking
- Provide administrative support to the clinical team
- Help maintain compliance with home health documentation requirements
QualificationsPreferred
- Home health office experience
- Knowledge of OASIS and CMS-485 documentation
- Experience using Synergy software
- Medical office or healthcare administrative experience
