What You Will Do:
Coordinate and provide expert care for patients with wound, ostomy, and continence needs. Educate patients, families, and staff while guiding care plans that foster independence and healing.

Coordinates patient assessment through collection of patient’s health data.

Analyzes patient assessments to determine nursing diagnoses and identifies expected outcomes.

Coordinates an interdisciplinary plan of care that prescribes interventions to attain expected outcomes.

Implements the interventions identified in the plan of care.

Evaluates the patient’s progress toward attainment of outcomes.
-
Performs patient assessment, develops plan of care and implements treatments for patients with selected disorders.

Serves as primary educator for patients/significant others and staff regarding W.O.C. issues.

Demonstrates proficiency and knowledge base of ostomy, wound, and continence management.
