Elevate Therapy is seeking a compassionate and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP/APRN) to join our growing outpatient mental health practice serving Connecticut and New York clients through in-person and Telehealth services.
We are looking for a provider who values thoughtful, relationship-based psychiatric care and enjoys collaborating with therapists to support clients through a comprehensive, whole-person approach.
We are looking for a Provider who:
- Has completed independent licensure requirements and does not require physician collaboration
- Is licensed as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in Connecticut
- Is comfortable working in a private practice setting as an independent contractor (1099), or interested in transitioning into private practice
- Has experience conducting psychiatric evaluations, providing medication management, and treatment planningValues a comprehensive approach that considers biological, psychological, and environmental factors affecting mental health
- Is knowledgeable in conductingADHD evaluations and ordering appropriate labs when clinically indicated
- Is interested in working with children, adolescents, and/or adults
- Collaborates effectively with therapists and other healthcare providers to provide coordinated care
- Demonstrates compassion, professionalism, strong communication skills, and clinical excellence
Role responsibilities:
- Conduct psychiatric evaluations and follow-up medication management appointments
- Develop individualized treatment plans and document patient progress
- Prescribe and manage medications as medically necessary
- Order and review labs as medically necessary
- Collaborate with other care providers to support integrated patient care
- Maintain timely and accurate clinical documentation
