About Memory Care Wellness Coordinator LPN the Role
We are seeking a compassionate and experienced Memory Care Wellness Coordinator LPN to join our team at Fountains at Mill Cove in Jacksonville, Florida. As a Memory Care Wellness Coordinator LPN, you will play a vital role in providing exceptional care and support to our residents with memory impairment. Your primary responsibility will be to coordinate and implement personalized wellness programs that promote physical, emotional, and social well-being.
Memory Care Wellness Coordinator LPN Responsibilities:
Develop and implement individualized wellness plans for residents with memory impairment, incorporating therapeutic activities, exercises, and social events
Collaborate with the interdisciplinary team to ensure comprehensive care and support for residents
Conduct assessments and evaluations to identify resident needs and monitor progress
Design and lead group activities, such as art therapy, music therapy, and exercise classes
Provide education and support to residents, families, and caregivers on memory care strategies and techniques
Maintain accurate records and reports on resident progress and care plans
Memory Care Wellness Coordinator LPN Requirements:
Current LPN license in the state of Florida
Prior experience in a long-term care setting, preferably in memory care
Strong understanding of memory impairment and its effects on daily life
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
