We are seeking a Physical Therapist to join a modern skilled nursing and rehabilitation center in Los Gatos, CA. The facility offers short-term rehab, long-term care, memory support, and restorative programming.
As the PT, you’ll perform evaluations, design individualized care plans, and lead therapeutic interventions focused on promoting independence, improving mobility, and optimizing patient outcomes. You will work in a supportive setting with organized workflows and strong interdisciplinary communication.
Conduct PT evaluations and functional mobility assessments.
Develop treatment plans tailored to patient needs.
Provide therapeutic activities, gait training, strengthening, and balance programs.
Document all sessions and progress accurately.
Communicate care updates with the interdisciplinary team.
Educate patients and caregivers on safe practices and home programs.
Follow evidence-based PT standards.
