Typical Working Conditions:Requires full range of body motion as well as standing and walking for extensive periods of time. Requires the ability to work confidently through critical situations and occasionally outside scheduled hours to accommodate patient needs. Potential exposure to communicable diseases, bodily fluids, toxic substances, medicinal preparations and other conditions common to a clinical environment. Occasionally lift/carry items weighing up to 25 pounds.
Work Location: As an employee of Family Health Network of Central New York, Inc. you may be assigned to any of our locations – Cortland Family Practice, Homer Family Practice, Marathon Health Center, Cincinnatus Health Center, Moravia Health and Dental Center, Administration, School Health Programs at Marathon, Cincinnatus, or DeRuyter Schools, or our mobile medical unit.
Qualifications: Graduation from an accredited school of Practical Nursing. Valid New York State Practical Nurse License. CPR (BLS) Certification and New York State Infection Control course completion.
Primary Function: Provide professional, empathetic nursing care for FHN patients following established standards and practices set up by the profession and FHN Policies and Procedures.
General Summary of Duties:
· Perform general outpatient nursing care to patients including venipunctures under a provider order.
· Active participant in Dramatic Performance Improvement model of patient care.
· Administer prescribed medications and treatments in accordance with the Policies and Procedures of Family Health Network of CNY Inc.
· Assist with the preparation of equipment and aid providers during treatment, examination, and testing of patients.
· Observe, record, and report patient’s condition and reaction to medications administered and treatments to provider.
· Ensure preferences are given to patients in emergency situations.
· Greet patients and prepare them for provider examination. Screen patients for necessary information and complete appropriate forms within the EHR. Instruct patients in the collection of samples and tests as ordered by provider.
· Arrange for all ordered patient testing and referrals.
· Respond to and refer incoming telephone calls. Collect all information needed for provider direction. Instruct patients and family regarding medication and treatment instructions per provider instruction.
· Huddle participation with care coordinator and provider to ensure proactive start to the day.
· Prepare clinical exam rooms including restocking and monitoring for outdates.
· Perform required in-house lab testing as ordered by provider, according to CLIA and Family Health Network protocol.
· Ensure proper maintenance and cleaning of all equipment and report faulty equipment to supervisor as per Family Health Network Policy and Procedure.
· Work with care coordinator to assist in patient self-management activities for patients with chronic conditions.
· Monitor all sample medications for outdates on a monthly basis and accurately maintain sample log book.
· Follow up on lab and radiology tracking reports weekly and per Family Health Network Policy and Procedure.
· Utilize patient education resources and provide education to patients per provider direction.
· Attend required meetings and participate in committees as requested. Participate in professional development activities and maintain professional affiliation.
· May be asked to participate in quality improvement activities which include but are not limited to performing PDSA and the follow up on high risk patients based on population reports for patients with poor clinical outcomes or with lapses in care.
· Requires compliance with Health Center’s written standards, including its Standards of Conduct and policies and procedures pertaining to all operational aspects, furthermore this position is required to report any suspected instances of noncompliance.
· LPN is part of the patient care team. The care team is comprised of the patient, care coordinator, nurse, and provider. As part of the patient care team, the LPN will engage patients in their own health care, including supporting patients and their families in self-management and positive behavior change. This will require good communication which is both open and supportive.
· Site rotation for work assignments as necessary.
· This position will function as a contributing member of the emergency preparedness team under the incident command system.
· Maintain strictest confidentiality at all times adhering to all required HIPAA Policies and Procedures.
· All other duties as assigned.
Pay: $25.00 - $32.00 per hour
