RN Case Manager – PACE Program

Summary:
Join our interdisciplinary PACE team as an RN Case Manager, supporting frail seniors to live safely and independently in the community. You will conduct assessments, coordinate care, manage clinical needs, and work closely with participants, families, and providers to deliver comprehensive, person-centered care.
Key Responsibilities:
- Complete nursing assessments and contribute to individualized care plans.
- Coordinate medical, social, home care, and rehabilitative services.
- Monitor participant health status and adjust care plans as needed.
- Provide direct nursing care, education, and home visits.
- Manage care transitions (hospital, SNF, home).
- Maintain accurate documentation and ensure regulatory compliance.
- On-call responsibilities.
Qualifications:
- Current RN license (state of Michigan).
- 1+ year of nursing experience (geriatrics, home health, or case management preferred).
- Strong communication, critical-thinking, and coordination skills.
- Valid driver’s license and ability to travel locally.

Compensation: $90-$95 annually
Pay: $90,000.00 - $95,000.00 per year
