Job Title: RN – Community Hospice Case Manager
Location:Grand Junction, Colorado
13-Week Contract
Day Shift: Monday–Friday | 8:00 AM – 5:00 PM
Pay Rate:$56/hourly
Mandatory Qualifications
- Active Colorado Registered Nurse (RN) License (required)
- Minimum 2 years of Registered Nurse (RN) experience required
- Previous Hospice, Home Health, Palliative Care, Case Management, Medical-Surgical, or Community Nursing experience preferred
- Graduate of an Accredited Nursing Program (ADN or BSN)
- Current BLS (Basic Life Support) / BCLS Certification required
- Experience managing an assigned patient caseload preferred
- Comfortable providing care in private homes, assisted living facilities, nursing homes, and group homes
Roles and Responsibilities
- Provide compassionate, high-quality hospice nursing care to patients in community and home-based settings
- Manage an assigned caseload of hospice patients and coordinate individualized care plans
- Perform comprehensive patient assessments and ongoing clinical evaluations
- Develop, implement, and update individualized care plans based on patient needs and goals
- Administer medications and treatments in accordance with physician orders and hospice standards
- Provide pain management and symptom control for patients with life-limiting illnesses
- Educate patients, families, and caregivers on disease progression, medications, symptom management, and end-of-life care
- Support patients and families with emotional, physical, and psychosocial needs
- Coordinate care with physicians, social workers, chaplains, nursing assistants, therapists, and other interdisciplinary team members
- Maintain accurate and timely patient documentation in the electronic medical record (EMR)
- Ensure compliance with hospice regulations, infection prevention standards, and organizational policies
- Participate in care conferences and interdisciplinary team meetings
- Provide occasional weekend backup and on-call coverage as scheduled
- Advocate for patient comfort, dignity, quality of life, and family-centered care throughout the hospice journey
