Position Summary
The Foster Care Case Manager is responsible for coordinating and monitoring foster care services to ensure the safety, well-being, and permanency of children placed in care. This position requires strong organizational skills, professional judgment, attention to detail, and the ability to manage multiple priorities while maintaining compliance with Colorado Department of Human Services regulations.
The successful candidate will serve as an advocate for children and families while ensuring all licensing, placement, documentation, and case management requirements are completed accurately and timely.
Essential Responsibilities
- Monitor foster care placements to ensure safety, stability, and regulatory compliance.
- Conduct regular home visits with foster families and children.
- Complete home studies and assessments for prospective foster parents.
- Develop and maintain positive working relationships with foster families.
- Coordinate services for children and families to support successful placements.
- Serve as a liaison between county departments, schools, therapists, courts, medical providers, and foster families.
- Maintain accurate, timely, and audit-ready case documentation.
- Assist with child intake, placement, and transition planning.
- Participate in multidisciplinary team meetings, staffings, court-related activities, and case reviews.
- Provide crisis intervention and support when necessary.
- Participate in the agency's on-call rotation schedule.
QualificationsRequired
- Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, Behavioral Sciences, or a closely related field;
OR
- Bachelor's Degree in a non-related field plus a minimum of two years of relevant experience supervised by a Master's-level professional in accordance with Colorado Department of Human Services requirements.
- Valid driver's license and reliable transportation.
- Ability to pass criminal background screening and drug screening.
- Strong written and verbal communication skills.
- Excellent organization, time management, and documentation abilities.
- Ability to work independently and exercise sound professional judgment.
Preferred
- Previous foster care, child welfare, case management, social services, or family services experience.
- Knowledge of Colorado foster care regulations and licensing requirements.
- Experience conducting home studies and family assessments.
- Experience working with children who have experienced trauma.
Schedule
- Monday through Friday
- 9:00 a.m. – 5:00 p.m.
- Participation in evening, weekend, holiday, and emergency calls
