The Care Facilitator is responsible for providing primary care of Intensive Outpatient clients including supervision and daily living services and ensuring the delivery of high-quality therapeutic services for each client.
Tasks are performed under the direction of the VP of Lotus Clinics & Clinical Compliance. Work performance is reviewed through informal supervision, formal supervision, reports, and evaluations. Incumbent to this position is a KidsTLC employee and subject to the KidsTLC personnel policies and procedures.

- Supervise Intensive Outpatient clients to ensure the daily physical and emotional needs of the youth are being fulfilled.
- Assist the Intensive Outpatient Director and therapeutic case coordinators with the overall operation of Intensive Outpatient services.
- Assist in ensuring compliance with all COA standards and ensuring continuous quality improvement.
- Monitor and maintain high quality record keeping and professional accountabilities for Intensive Outpatient Services.
- Assist with contacting parents for follow up and review of their childs progress in the Intensive Outpatient Program.
- Assist in professional growth maintenance.
- Utilize the Philosophy of Care of Phoenix and trauma-based interventions as directed.
- Perform other duties as assigned or requested.
- This position has no direct reports.
QUALIFICATIONS

High school diploma or equivalent
College credit hours in social work, psychology, or a related mental health field from an accredited college, preferred.
Must be at least 21 years of age with a valid drivers license and satisfactory driving record.
