- Participates in the evaluation and performance review of field staff.

- Contact field personnel such as nurses, physical therapists, occupational therapists, and home health aides, homemaker and schedule appropriately skilled persons to staff the case.
- Answer, screen, Log In to report and forward incoming phone calls.

- Take and relay messages between patients and homemakers.
- Effectively document patient demographics, pertinent medical history and insurance information in the patient Electronic Medical Record (EMR).
- Upload documents to HSRM (as needed) POC, SN DC Summary, PO, Therapy POC, Homemaker POC, and Homemaker Timesheets.
- Track documents requiring MD Signature on HSRM
- Obtain required documentation from referral sources prior to patient admission
- Contact field personnel such as nurses, physical therapists, occupational therapists and home health aides and schedule appropriately skilled persons to staff the case.
- Responsible for monitoring and scheduling all Skilled and Non-Skilled tasks and scheduling appropriately according to frequency and duration.
- Create Communication for any incidents between patients, homemakers, VA.
- Routinely checks for receipt of referrals on fax machine and voicemail. Accesses ECIN/Allscripts system to manage new referrals from facilities using the ECIN/Allscripts referral system.
- Communicates effectively with field staff when interacting with them concerning matters of scheduling and care coordination
