Responsibilities
Coordinate and oversee the daily operations of the Sub-Acute Unit, including patient admissions, transfers, and discharges
Develop and implement effective care plans that meet the unique needs of each resident
Collaborate with the interdisciplinary team to ensure seamless communication and coordination of care
Monitor and report patient progress, identifying areas for improvement and implementing changes as necessary
Provide exceptional customer service to residents, families, and visitors, addressing their concerns and needs in a timely and professional manner
Maintain accurate and up-to-date records, including patient charts, treatment plans, and billing documentation
Participate in quality improvement initiatives to continuously improve patient care and outcomes
