Member Duties : In this role, AmeriCorps Members will be Patient Navigators supporting the Community Health Workers by performing telephonic outreach to families with medically complex children in NYC. Members will: • Conduct screenings for Health-Related Social Needs (HRSNs) • Facilitate referrals and connect families to essential services • Provide care coordination in collaboration with hospital partners, Social Care Networks (SCNs), and community-based organizations By working directly with community members, you will help them achieve better health outcomes.
