Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs using physiology and anatomy knowledge.
- Develop and implement tailored care plans that incorporate evidence-based practices such as trigger point therapy and orthopedics techniques.
- Assist patients with discharge planning by coordinating care transitions across inpatient, outpatient, home health, and community settings.
- Provide hands-on therapy including manual techniques, therapeutic exercises, and modalities to promote healing and mobility.
- Monitor patient progress regularly, adjusting treatment strategies to optimize recovery outcomes.
- Document all patient interactions accurately within Electronic Medical Record (EMR) systems while ensuring compliance with medical terminology standards.
- Collaborate with multidisciplinary teams to support patient care goals in hospital, outpatient clinic, or home & community care environments.
Qualifications
- Valid licensure as a Physical Therapist (DPT) with current registration in the practicing region.
- Demonstrated experience in inpatient hospital settings, outpatient clinics, or home health environments, with experience treating spinal conditions
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy.
- Proven ability to develop effective care plans for diverse populations such as pediatrics and geriatrics.
- Experience with discharge planning and managing care across acute care and community settings.
- Familiarity with EMR systems for documentation and patient record management.

Pay: $45.00 - $55.00 per hour
