Job Summary
We are seeking a dedicated and compassionate Nurse to join our healthcare team to act as a case manager and perform in person assessments. Though our census is more or less clustered, we intend on growing an serving in many areas throughout the state of New Jersey. The ideal candidate will be comfortable being on the road and seeing patients in home performing required assessments and updating care plans. The right individual will work hand in hand with our seasoned DON. As the clinical operation grows they will likely be assigned a region, but that is part of the bigger picture.
Duties
- Assess patients timely.
- Develop individualized care plans for caregivers to follow as needed.
- Maintain accurate clinical charts.
- Be in field
Qualifications
- An RN License is required
- At least 1 year of home care, hospice or travel nurse experience is highly preferred.
- Bilingual is a plus
- Reliable means of transportation
- Fairly tech savvy
- Strong interpersonal skills are required for effective communication with patients, families, and healthcare professionals.
- Positive attitude an absolute must
