Position Summary:
We’re seeking a Nurse Practitioner to provide primary and preventive care services in the comfort of patients’ homes. This role is ideal for a self-motivated, compassionate provider who thrives on autonomy and values continuity of care. Must have a flexible schedule and a passion to provide high quality care with excellent communication to the patients they serve.
Key Responsibilities:
- Conduct comprehensive health assessments and physical exams in patients’ homes
- Manage chronic and acute conditions
- Perform diagnostic testing and interpret results
- Educate patients and caregivers on medications, disease prevention, and healthy living
- Coordinate with physicians, specialists, and care teams as needed
- Document all visits and maintain accurate, up-to-date patient records
- Travel locally to patient residences
Qualifications:
- Licensed Nurse Practitioner in the state of Hawaii
- Board certification in Family, Adult-Gerontology, or equivalent specialty
- Minimum of 2 years of clinical experience preferred
- Valid driver’s license and reliable transportation
- Strong communication, time management, and clinical judgment skills
- Experience with electronic health records (EHR) and mobile documentation tools
