Occupational Therapist
- Location: 1441 Powell St, San Francisco, CA 94133,
- Schedule: Full-time | Monday–Friday | 40 hours/week | 8:00 AM–4:30 PM
- No weekends
- Pay Range: $53.55–$71.40/hour
- Setting: Outpatient + Home Care
We are seeking an Occupational Therapist to evaluate our patients physical condition and functional status to establish a realistic, measurable goal and set up a treatment plan.
- Provides Occupational Therapy treatment & interventions to On Lok participants.
- Provides training, guidance, and supervision to Rehabilitation Aides and participant/family member in carrying out the Functional Maintenance Program.
- Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem solving, and processes.
- Fulfills all administrative requirements, including but not limited to center-based supervision of rehabilitation department in collaboration with PT, attendance at team & staff meetings, and recording & tracking of OT rehabilitation equipment.
QUALIFICATIONS (knowledge, skills, abilities):
- Graduate of an approved Occupational Therapy program.
- Successful completion of certification exam & initial registration through National Board for Certification in Occupational Therapy (NBCOT).
- Licensed in the state of California through California Board of Occupational Therapy (CBOT).
- Two years experience in physical disabilities, with focus in working with the geriatric or frail population required.
- Flexible, self-motivated, and resourceful.
- Ability to work independently, and as part of an interdisciplinary team.
- Physical requirements include ability to lift/move up to 75 pounds occasionally, and up to 50 pounds frequently.
- Physical activities require the ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repetitive motions of the wrist, hands, and fingers.

The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.We understand the importance of a system that works together.
