Job Summary

Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates while maintaining accurate medical documentation in Electronic Health Record (EHR) systems.
- Observe patient conditions closely, noting any changes or concerns related to dementia, Alzheimer's disease, or other cognitive impairments; communicate findings to supervising healthcare providers promptly.
- Administer medications and perform catheter care following prescribed instructions while adhering to HIPAA regulations to ensure patient confidentiality.
- Support patients with meal preparation, feeding, light cleaning, laundry, and maintaining a safe and clean environment conducive to health and comfort.
- Provide specialized care for individuals with developmental disabilities or those requiring hospice or memory care services.
- Use medical terminology accurately when documenting patient interactions and updates in medical records; work effectively within home & community care settings.
- Employ EMR/EHR systems efficiently for scheduling, documentation, and communication with healthcare teams.
- Assist with mobility and transfers using appropriate equipment like Hoyer lifts or gait belts to promote safety.
- Support family members by providing education on patient care needs and ensuring compliance with care plans.
Qualifications
- Prior experience in home health care, nursing homes, assisted living facilities, or long-term care environments is highly desirable.
- Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and infection control practices.
- Familiarity with medical terminology, medical documentation standards, and EMR/EHR systems.
- Certification in CPR is required; additional certifications such as hospice care training or dementia/Alzheimer’s certification are a plus.
- Experience working with individuals with disabilities or developmental challenges enhances your ability to deliver personalized care.
- Ability to perform physical tasks such as lifting patients safely using Hoyer lifts or other assistive devices; capable of standing and walking for extended periods.
