The RN Supervisor is responsible for providing clinical leadership and supervision of nursing staff and ensuring safe, high-quality care for residents/patients in the Skilled Nursing and Subacute units. The RN Supervisor oversees daily nursing operations, coordinates patient care, supports staff, and ensures compliance with facility policies, regulatory requirements, and standards of nursing practice. 12 hours shifts Full time
Essential Duties and Responsibilities:
- Supervise and coordinate the activities of RNs, LVNs/LPNs, CNAs, and other nursing personnel during the assigned shift.
- Assess residents/patients and identify changes in conditions requiring immediate intervention or provider notification.
- Provide direct nursing care when needed and assist staff with complex clinical situations.
- Oversee care of subacute patients requiring higher-acuity services, including IV therapy, wound care, enteral feeding, tracheostomy care, respiratory treatments, and other skilled services within the nurse's scope of practice.
- Review and follow physician orders, treatment plans, medication administration, and nursing interventions.
- Ensure accurate and timely documentation in the electronic medical record.
- Communicate significant changes in residents' conditions to physicians, nurse practitioners, families, and the interdisciplinary team as appropriate.
- Respond appropriately to emergencies and initiate emergency protocols when necessary.
- Monitor medication administration and treatment procedures for compliance with physician orders and facility policies.
- Conduct rounds and monitor the overall condition, safety, and quality of care provided to residents.
- Assist with admissions, readmissions, transfers, and discharges, including completion and review of required documentation.
- Ensure appropriate staffing and assignment of nursing personnel based on patient acuity and staffing needs.
- Provide orientation, education, coaching, and performance feedback to nursing staff.
- Address staff concerns and resolves clinical or operational issues in collaboration with nursing management.
- Participate in quality assurance/performance improvement (QAPI) activities and help identify opportunities to improve patient outcomes.
- Ensure compliance with applicable federal and state regulations, including CMS, CDPH, and facility policies, as applicable.
- Monitor infection prevention and control practices and reinforce appropriate precautions.
- Participate in incident/accident reporting, investigation, and follow-up as required.
- Maintain effective communication with physicians, rehabilitation staff, dietary services, social services, pharmacy, laboratory, and other departments.
- Promote resident rights, dignity, privacy, and person-centered care.
- Serve as the nursing administrative/clinical resource for the assigned shift.
- Perform other duties as assigned by the Director of Nursing or Administrator.
Qualifications:
- Current, unrestricted RN license in the state of employment.
- Associate Degree in Nursing required; BSN preferred.
- Previous experience in skilled nursing, subacute care, rehabilitation, or acute-care nursing preferred.
- Previous supervisory or charge nurse experience preferred.
- Current BLS certification.
- Strong clinical assessment, leadership, communication, and organizational skills.
- Ability to manage multiple priorities and respond effectively to changes in patient condition.
- Knowledge of applicable nursing regulations, infection control, medication management, documentation, and patient safety standards.
Physical/Work Requirements:
- Ability to work effectively in a fast-paced healthcare environment.
- Ability to work weekends, holidays, and varying shifts as required by facility operations.
