PRN Licensed Practical Nurse (LPN) – Pediatric Home Care (GAPP)

Modani Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a pediatric client in the home through Georgia's GAPP (Georgia Pediatric Program).
This position is ideal for an LPN who enjoys providing individualized care and building meaningful relationships with clients and families.
Schedule Information
- This is a PRN position based on the client's Medicaid-authorized nursing hours.
- The current assignment offers 8 hours per week.
- The work schedule is determined by the client's approved schedule and care needs. While we strive to accommodate employee availability whenever possible, the client's schedule must remain the priority.
- As with all PRN positions, continued hours depend on the client's ongoing authorization and need for services.
- Additional opportunities, such as supervisory visits or future pediatric assignments, may become available as our program continues to grow, but cannot be guaranteed.
Responsibilities
- Perform comprehensive head-to-toe nursing assessments and monitor for changes in the client's overall condition.
- Assess neurological status related to spina bifida and hydrocephalus, including mobility, strength, sensation, balance, gait, headaches, dizziness, and other signs of neurological decline.
- Monitor respiratory status, vital signs, and observe for signs of respiratory distress or other acute changes.
- Assess gastrointestinal status, monitor bowel function, and assist with bowel management interventions as ordered.
- Monitor and manage neurogenic bladder care through a Mitrofanoff, including catheterization, urinary output, urine characteristics, and signs of urinary tract infection or catheter-related complications.
- Perform wound assessments and ordered wound care for an open spinal wound while monitoring skin integrity and healing.
- Administer prescribed medications and monitor for effectiveness, side effects, and adverse reactions.
- Reinforce infection prevention, catheter care, wound care, medication compliance, and other aspects of the prescribed plan of care with the client and caregiver.
- Communicate significant changes in condition to the supervising RN and physician.
- Complete accurate and timely documentation in accordance with GAPP and agency standards.
Qualifications
- Current Georgia LPN license in good standing
- Current CPR certification
- Valid Georgia driver's license
- Reliable transportation and current auto insurance
- Pediatric, home health, or GAPP experience is required
- Comfortable working independently in a home setting while collaborating with the clinical team
