Enhanced Care Management (ECM) Case Manager (RN)
Program Type: Enhanced Care Management (ECM) — Care Coordination Services
Schedule: Monday – Friday | Full-Time (40 hours/week)
We are seeking a dedicated and compassionate Enhanced Care Management (ECM) Case Manager (RN) to join The Good Seed team in Leimert Park, CA. This position supports the ECM program by engaging members, facilitating enrollment, and ensuring continuity of care through coordinated clinical and social service interventions.
The ECM Case Manager is responsible for developing and implementing comprehensive, patient-centered care plans that address both medical and psychosocial needs. This role focuses on improving health outcomes by supporting individuals with complex needs through consistent outreach, care coordination, and multidisciplinary collaboration.
Engage members in their living environments or preferred care locations
Conduct comprehensive risk assessments and develop individualized, patient-centered care plans
Implement and update care plans at least quarterly or as needed
Support health behavior change using motivational interviewing and trauma-informed care practices
Educate members on self-management skills and involve caregivers when appropriate
Monitor treatment adherence and care plan progress
Coordinate with hospitals and providers on discharge planning and transitions of care
Connect members to housing, transportation, and other social services
Participate in or initiate case conferences with interdisciplinary care teams
Maintain at least one in-person member contact per month
Document all care management activities in the EHR with appropriate coding and tracking
Accompany members to appointments as needed
Maintain regular communication with assigned members
Perform other duties as assigned
Required:
High school diploma required; Bachelor’s degree in Social Work, Nursing, or related field preferred
RN license (or relevant healthcare certification as applicable)
2–3 years of experience in community health, social services, or care coordination
1–3 years of case management or care coordination experience
Valid driver’s license and ability to travel locally (SPA 6 and SPA 8)
Strong communication and interpersonal skills
Ability to work effectively in interdisciplinary team settings
Proficiency in Microsoft Office Suite
Preferred:
Experience with Electronic Health Records (EHR) systems
Bilingual in English and Spanish or Armenian
Experience working in Enhanced Care Management (ECM) or similar programs
Strong independent problem-solving and resource navigation skills
This position operates in both office and community-based settings across SPA 6 and SPA 8 in Los Angeles, CA, with significant field-based work and travel required (up to 75%).
Candidates must be comfortable working in dynamic environments, traveling frequently to client locations, and maintaining professionalism in high-need, complex care situations. The role requires strong emotional resilience and a commitment to trauma-informed, patient-centered care.
