What You Will Do
- Coordinate end-of-life care for hospice patients and families across all care settings, maintaining regular patient/family contact and individualized care plans.
- Educate patients and caregivers on disease progression, symptom management, hospice philosophy, and end-of-life expectations.
- Assess patient needs, monitor condition changes, administer medications/treatments, and update physicians and care plans accordingly.
- Lead and collaborate with the Interdisciplinary Group (IDG) to develop, implement, and revise individualized plans of care.
- Coordinate services, referrals, medical equipment, supplies, and community resources to ensure comprehensive, continuous care.
- Complete timely, accurate electronic health record (EHR) documentation in compliance with agency, accreditation, state, and federal standards.
- Supervise Home Health Aides and LPNs, support discharge planning, mentor new staff, and provide age-appropriate, patient-centered nursing care.
Schedule Details
- Full time
- Monday through Friday 0800-1630.
- Salaried position.
- No weekend or on call commitment.
- Holiday rotation required (2 holidays per year).
What We Are Looking For
- Registered Nurse license issued by the state in which the team member practices, and
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained upon hire unless department leader has determined it is not required, and
- A valid driver’s license issued by the Division of Motor Vehicles.
- Completion of an accredited or approved program in Nursing.
- Typically requires 1-2 years of experience in nursing that includes experiences in home health, hospice, medical/surgical, long term care, emergency care/ICU or oncology or similar related experience.
