We are seeking a Residency-trained Clinical Pharmacist

- $80–$116/hour
- $10,000 Sign-On Bonus

Join a respected 400+ bed acute care hospital / Academic Medical Center
YOUR ROLE:
- Interdisciplinary patient rounds
- Work decentralized as a Clinical Pharmacist
- Verify and review physician medication orders
- Evaluate medication therapy for safety, efficacy, allergies, interactions, and appropriate dosing
- Perform pharmacokinetic dosing and therapeutic drug monitoring
- Manage anticoagulation therapy
- Complete renal dose adjustments and IV-to-PO conversions
- Compound and accurately dispense medications
- Provide drug information and clinical recommendations to physicians, nurses, and patients
- Support formulary management and therapeutic interchange initiatives
- Document clinical interventions and medication safety activities
- Promote safe, evidence-based medication practices throughout the hospital
QUALIFICATIONS:
- Doctor of Pharmacy (PharmD)
- PGY1 Residency preferred, or at least one year of acute care hospital pharmacy experience
