Position Overview (Licensed Practical Nurse – LPN)
Polaris Rehabilitation & Care Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our clinical team. In this role, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and support the delivery of safe, high-quality, resident-centered care.
As an LPN, you will work closely with Registered Nurses, nursing assistants, physicians, and other members of the interdisciplinary care team. This position is ideal for a nurse who values hands-on care, meaningful relationships with residents, and a collaborative environment focused on positive clinical outcomes.
Key Responsibilities
Deliver direct nursing care in accordance with individualized care plans, physician orders, and established clinical standards
Monitor resident conditions and promptly report significant changes to the Registered Nurse, physician, or appropriate clinical team member
Administer medications, treatments, and therapeutic interventions accurately and within scope of practice
Assist with care coordination, resident assessments, and daily clinical operations
Supervise, delegate, and support Certified Nursing Assistants and other direct care staff
Communicate effectively and respectfully with residents, families, physicians, and interdisciplinary team members
Maintain accurate, timely, and compliant documentation within the electronic medical record system
Follow infection prevention, medication administration, safety, and emergency response protocols
Respond appropriately to urgent resident needs and changes in condition
Promote resident dignity, comfort, independence, and quality of life
Maintain compliance with facility policies and applicable federal and Wyoming regulations
Participate in quality improvement, resident safety, and care planning initiatives as assigned
Qualifications
Graduate of an approved practical nursing program
Current, active Licensed Practical Nurse (LPN) license in good standing in the State of Wyoming
Current CPR or Basic Life Support certification
Previous experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred
Strong clinical observation, critical thinking, and problem-solving skills
Ability to work effectively within a collaborative clinical team
Excellent communication and interpersonal skills
Ability to organize and manage multiple responsibilities in a fast-paced healthcare environment
Strong attention to detail and commitment to accurate documentation
Dedication to providing compassionate, high-quality, resident-centered care
