Job Summary
We are seeking a dedicated and compassionate Hospice LPN Patient Care Coordinator to join our interdisciplinary healthcare team. The ideal candidate will possess a strong background in nursing, with extensive experience in hospice, acute, and post-acute care settings. As a vital member of our organization, the Patient Care Coordinator will oversee patient assessments, coordinate care plans, and ensure the delivery of high-quality hospice services. This role requires excellent communication skills, clinical expertise, and a commitment to providing empathetic patient-centered care in a collaborative environment.
Coordinates and facilitates the provision of high-quality, cost-effective hospice services through efficient triaging and communication. Supports clinical management to ensure appropriate utilization of resources as well as patient safety and satisfaction
Schedule:
- Part-time, 24 hour days (3) 8-hour shifts - 7:30am - 4:00pm
- Rotating weekends

Responsibilities
- Conduct comprehensive patient assessments to evaluate clinical needs and develop personalized care plans aligned with hospice protocols.
- Coordinate interdisciplinary team efforts to ensure seamless delivery of hospice services, including symptom management and emotional support.
- Manage patient intake processes, including documentation and verification of eligibility for hospice care.
- Monitor patient progress regularly, adjusting care plans as necessary to optimize comfort and quality of life.
- Collaborate with healthcare professionals across hospital, home health, and outpatient settings to facilitate discharge planning and post-acute care transitions.
- Maintain accurate medical records utilizing Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations.
- Provide education to patients and families regarding disease progression, medication management, and available support services.
- Participate in quality assurance initiatives, ensuring adherence to health regulation policies and best practices in hospice care.
Skills
- Extensive clinical experience in hospice nursing with familiarity in critical care, ICU, emergency medicine, or trauma center environments.
- Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques relevant to end-of-life care.
- Proficiency in EMR/EHR management systems coupled with understanding of health regulation policies such as HIPAA.
- Exceptional patient service skills with the ability to communicate effectively with diverse populations including geriatrics and pediatric patients.
- Ability to collaborate effectively with healthcare professionals across hospital medicine, behavioral health, managed care, and outpatient clinics.
- Experience managing case management activities such as discharge planning, utilization management, and clinical case oversight.
- Knowledge of medical-surgical procedures, infectious disease care, post-acute care processes, and home health services.
- Strong organizational skills for managing multiple patients simultaneously while maintaining attention to detail in documentation and patient assessments.
