What You'll Do
Clinical Nursing
- Provide skilled, in-home nursing visits (assessment, care planning, teaching, coordination).
- Perform clinical interventions within scope (medication management/education, disease management coaching, wound and skin assessments, vitals monitoring, safety checks).
- Develop, implement, and update care plans; collaborate with clients, families, providers, and internal care teams.
- Evaluate outcomes and adjust interventions to support client goals and independence.
- Educate and empower clients/families on condition management, medications, and safety.
- Complete timely, accurate documentation per policy and Oklahoma regulations.
Administrative & Scheduling Oversight
- Create, maintain, and oversee caregiver and clinician schedules, including VA-authorized visits, ensuring alignment with care plans and clinician availability.
- Serve as the point of contact for day-to-day schedule changes, same-day call-offs, and urgent reassignments across multiple service lines.
- Build and maintain rapport with caregivers, clinicians, and clients to support consistent, reliable coverage.
- Manage client services as outlined in service agreements and care plans, escalating and resolving gaps in coverage.
- Supervise and mentor caregiving and scheduling staff, reinforcing best practices and Culture of Care standards.
- Maintain scheduling databases and reporting using scheduling software and Microsoft Suite.
- Support compliance with medical coding, authorization tracking, and documentation standards across service lines (home health, hospice, transportation, or others as applicable).
What It Takes
- Active Oklahoma RN license in good standing.
- 5+ year of recent clinical experience (home health, hospice, primary care, or hospital preferred).
- 5+ years of related scheduling/operations experience, ideally in home health, hospice, or multi-service-line environments, including VA authorizations.
- Strong assessment skills, clinical judgment, and a calm, teach-first approach.
- Working knowledge of basic medical terminology and medical coding (preferred).
- Familiarity with scheduling platforms (e.g., Axxess) and Microsoft Suite.
- Ability to manage competing priorities — clinical caseload and administrative/scheduling demands — with sound judgment under pressure.
- Comfort working independently in the home and traveling locally (valid driver's license, reliable transportation).
- Heart for relationship-based care and serving diverse adult populations, including Veterans.
Schedule & Location
- Full-time, primarily daytime hours with flexibility for schedule and clinical emergencies as they arise.
- Service area includes Eastern and Northeastern Oklahoma.
