Job Summary
We are seeking a compassionate and skilled Physical Therapist to join our Home Care team in Manchester, NH. The Physical Therapist provides in-home physical therapy services to patients of all ages, develops individualized treatment plans, evaluates patient progress, and collaborates with physicians and interdisciplinary care teams to deliver high-quality patient care. This is a full-time, day shift position that includes travel throughout southern New Hampshire.
Duties & Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop, implement, and modify individualized treatment plans.
- Reassess patient progress and update care plans as needed.
- Coordinate care with physicians, nurses, caregivers, and other healthcare professionals.
- Educate patients and families on treatment plans, exercises, mobility, and safety.
- Document evaluations, treatments, and patient progress accurately in the electronic medical record.
- Supervise Physical Therapist Assistants (PTAs) in accordance with New Hampshire regulations and agency policies.
- Maintain compliance with organizational policies, safety protocols, infection control standards, and ethical guidelines.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Travel throughout assigned service areas to provide home health physical therapy services.
- Participate in a rotating weekend schedule.
Requirements
- Graduate of an accredited Physical Therapy (APTA) program.
- Current New Hampshire Physical Therapist license.
- Current BLS certification.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Minimum 1 year of physical therapy experience.
- Basic computer and electronic documentation skills.
- Ability to travel throughout southern New Hampshire and participate in a rotating weekend schedule.
