Job Summary
We are seeking a compassionate and energetic CNA/Personal Assistant/Care Manager to provide exceptional care and support to our client. Duties
- Perform patient observation to monitor health status, vital signs, and behavioral changes, ensuring timely intervention when necessary
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods.
- Support clients with meal preparation, cooking, laundry, cleaning, and other light household tasks to promote a safe and comfortable environment
- Maintain strict confidentiality of patient information in compliance with HIPAA regulations and home healthcare standards
- Manage healthcare team to document & coordinate care plan, medical records, client needs, effectively through client communication strategies
Experience
- Prior experience working in private client home, assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
- Proven ability to navigate expertly Apple, IPhone, Google Docs, Notes, etc
- Demonstrated knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) including CPR certifications
- Proven ability to work with individuals with developmental disabilities or cognitive impairments.
- Experience managing patient transfers using various hands-on methods
- Familiarity with home health regulations, medical records management, and documentation procedures within EHR or EMR systems
- Hands-on caregiving experience for those requiring long-term care in home
- Ability to handle heavy lifting safely while providing patient transfer assistance or mobility support.
