Seasons Care Hospice is seeking compassionate and experienced RNs to join our team as Hospice Case Managers serving the Ellijay GA
Full-time positions with flexible scheduling.
Position Overview:
Our Hospice RN Case Managers plan, coordinate, and deliver patient-centered hospice care. This role includes assessing patient needs, developing care plans, implementing interventions, and collaborating with families and the interdisciplinary team to ensure comfort and quality of life.
Schedule:
- Full-time
- Weekend availability required (based on patient needs)
- Every other weekend (based on patient needs)
Qualifications:
- Active RN license
- Hospice experience preferred
- Strong clinical, organizational, and communication skills
- Compassionate, patient-focused approach to care
- Able to commute to diffrent locations.
