American Traveler is hiring a travel RN Case Manager for an acute inpatient care coordination role requiring at least 1 year of acute care nursing experience and knowledge of health care reimbursement systems.

Hospital-based inpatient case management setting

Acute inpatient patient population across medical/surgical, ED, and critical care service lines
-
Focuses on discharge planning, care transitions, and post-acute coordination including Home Health, DME, IV Infusion, SNF Sub-Acute, and Acute Rehab

Uses Epic EMR

Day shift: 8:30 AM – 5:00 PM (8-hour shifts)

Active VA or Compact RN license required; pending licenses not accepted

All certifications must be active at time of consideration; pending certifications not accepted

Minimum 1 year of experience in an acute medical/surgical, ED, or critical care nursing setting

Associate's Degree in Nursing (ADN) required at minimum

Experience using an electronic medical record system

Knowledge of health care reimbursement systems including Medicare Advantage, commercial payers, Medicaid, and CMS regulatory rules
-
Last 4 digits of SSN and DOB required for consideration

Completes initial face-to-face assessments for every admitted patient within 24 hours of admission to identify discharge needs
-
Collaborates with hospital-based physicians, nursing staff, utilization review, and the broader healthcare team to ensure care progression toward clinical and cost-effective outcomes

Coordinates referrals to in-network post-acute vendors and communicates transition plans with physicians, patients, and families

Documents discharge plans in accordance with applicable policies and regulatory requirements
