PeaceHealth is seeking a Registered Nurse Care Manager for a Part-Time (0.60 FTE), Day Shift at our critical access hospital in Sedro Woolley. Compensation: $42.96 - $79.55 per hour
The hiring rate is based RN experience.
This position is represented by a collective bargaining agreement. Multiple openings may be available.

As an RN Care Manager, you'll serve as a key advocate, partnering with physicians, multidisciplinary teams, community resources, and payors to create comprehensive care plans that improve outcomes and enhance the patient experience.

The RN Care Manager coordinates and facilitates discharge planning and care transitions for selected patient populations. This role promotes effective utilization of healthcare resources while supporting positive clinical, financial, and patient satisfaction outcomes. The Care Manager serves as a liaison between patients, families, healthcare providers, community partners, and third-party payors to ensure patients receive appropriate, timely care while meeting regulatory and reimbursement requirements.
- Review admission information and determine appropriate levels of care using established clinical criteria.

Develop, implement, and monitor individualized discharge plans in partnership with patients, families, and the interdisciplinary care team.

Assess patients' clinical, psychosocial, environmental, and financial needs to identify barriers and opportunities for successful care transitions.

Coordinate post-discharge services and community resources to support continuity of care.

Partner with physicians and care teams to ensure accurate documentation supporting medical necessity and reimbursement.

Facilitate interdisciplinary care conferences aimed at improving quality, reducing costs, and enhancing patient satisfaction.

Collaborate with Patient Financial Services to support accurate documentation and reduce claim denials.

Conduct utilization reviews and monitor admission status using InterQual and other recognized criteria.

Communicate with insurance providers and third-party payors to obtain authorizations and address payment-related concerns.

Support palliative care initiatives as needed, including symptom management coordination, family meetings, comfort care planning, and POLST discussions.

Maintain required care management documentation and databases.
- Graduate of an accredited School of Nursing

Current Registered Nurse (RN) license in the state of practice

Basic Life Support (BLS) certification upon hire

Minimum of three years of clinical nursing experience in an acute care setting
Equivalent experience in skilled nursing facilities, outpatient case management, home health, or hospice may be considered.

Utilization Management and/or Quality Management experience

Specialty Certification

Knowledge of third-party reimbursement processes

Ability to prioritize and manage multiple responsibilities effectively

Proficiency with electronic medical records and Microsoft Office applications
